Job summary

Community Care Family Medicine is hiring a Full Time LPN to join our team. This 40 hour per week position involves vaccines, patient documentation, and maintaining smooth patient flow in a clinic setting.

The ideal candidate holds a NYS LPN license, has prior medical practice experience or as MA/PCT, and is proficient with EMR systems. We offer a comprehensive benefits package within a patient‑centered environment.

Qualifications

• Strong computer skills and EMR experience.

Responsibilities

Job description

Our Community Care Family Medicine office is looking for a Full Time LPN to join their team! This is a 40 hour per week position.

Responsibilities

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.
• Obtain specimens (i.e., urine, blood, etc.) from patients as directed by the provider.
• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Compensation

$26.00 - $33.00 per hour
