Job summary

Birch Creek Post Acute & Rehab in Tacoma, WA is seeking dedicated LPNs to join our clinical team and provide compassionate, high-quality care to residents in a long-term care and skilled nursing setting.

We offer full-time shifts: 6:00 AM–6:00 PM (12-hour rotating) and 6:00 AM–2:00 PM (8-hour). Pay ranges from $36.00 to $41.56 per hour with shift differentials, daily pay access, and comprehensive benefits.

Qualifications

• Current LPN license in Washington state.
• CPR/BLS certification (or willingness to obtain).
• Experience in LTC or SNF preferred; new grads welcome.

Responsibilities

• Provide nursing care to meet resident needs per standards and physician orders.
• Administer medications and treatments with precision and compassion.
• Collaborate with RNs, CNAs, and interdisciplinary teams.
• Support residents’ physical, emotional, and psychological well-being.
• Assist with resident comfort and care planning.
• Monitor patient status and maintain confidentiality per policies.

What You’ll Do:

• Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines

• Administer medications and treatments with precision and compassion

• Collaborate with RNs, CNAs, and interdisciplinary teams

• Support residents’ physical, emotional, and psychological well-being

• Assist with resident comfort.

• Collect samples and monitor catheters. Provide wound care.

• Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards.

• Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration.

• Help coach staff as necessary.

Requirements:

• Current LPN license in [State]

• CPR/BLS certification (or willing to obtain)

• Previous LTC or SNF experience preferred, but new grads welcome!

• Positive attitude, reliability, and strong clinical judgment
