Job summary

Reliant Care Management Company is seeking a dedicated LPN for full-time night shifts in Kentucky. You will assess residents, manage admissions and discharges, and lead the care team while following facility protocols and documenting changes.

Responsibilities include wound care, emergency response, and coordination with Behavioral Health Residents. A caring, ethical approach is essential to maintain high-quality resident care and teamwork.

• Provide wound care per protocols.
• Lead CNAs and CMTs ensuring compliance with facility policies.
• Respond to emergencies and support Behavioral Health Residents.

Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations

• Will be working with Behavioral Health Residents
