Job summary

Brookfield-Health-Care-Center in Missouri seeks an experienced LPN to join our team and provide compassionate resident care. You will perform assessments, admit patients, coordinate discharges, and lead CNAs while ensuring adherence to facility protocols and timely reporting of changes in patient condition.

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
