Job summary

Home Recovery Home Aid is seeking an LPN to deliver in-home nursing care, conduct patient visits, and coordinate with a multidisciplinary team to support patients in their homes.

The role requires an active LPN license, a valid driver’s license, and at least two years of nursing experience; it offers flexible full-time or part-time hours with weekly pay and comprehensive benefits.

Responsibilities

• Collaborate with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver home-based care.
• Assess, plan, direct, and evaluate each patient’s care plan.
• Manage emergency situations promptly and professionally.
• Complete medication administration and treatment needs for assigned patients.
• Provide nursing care in accordance with regulations and company policy.
• Prepare and maintain clinical and progress notes in a timely manner.

Responsibilities

What You Will Do:

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis
