Job summary

Home Recovery Home Aid is seeking compassionate LPNs to join our team providing in-home patient care across the Germantown area. You will collaborate with physicians, nurses, therapists and social workers to deliver coordinated care during home visits.

This role offers flexible hours (full-time or part-time), weekly pay, and comprehensive benefits including medical, dental, vision, life insurance, holidays and PTO.

Qualifications

• Active LPN state license.
• Valid driver's license.
• Two or more years of applicable nursing experience.
• Eligibility for certain benefits may depend on employment status.

Responsibilities

• Collaborate with physicians, nurses and therapists to deliver home-based care.
• Assess and plan each patient's care to meet needs.
• Manage emergencies promptly and professionally.
• Complete medication administration and treatment needs for assigned patients.
• Provide nursing care according to regulations and company policy.
• Prepare and maintain accurate clinical and progress notes.

Responsibilities

What You Will Do:

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis
