Job summary

CareRing Health in Germantown, MD seeks a compassionate Licensed Practical Nurse to provide in-home care. You will collaborate with physicians and support staff to deliver comprehensive nursing services to patients in their homes.

The role requires an active LPN license, a valid driver's license, and at least two years of nursing experience. You will assess patient needs, administer medications, document progress, and respond to emergencies promptly.

Responsibilities

• Deliver home-based nursing care in collaboration with physicians, nurses, and caregivers.

• Complete medication administration and treatment needs for all assigned patients.
• Provide nursing care in accordance with regulations and Company policy.
• Prepare and maintain accurate clinical and progress notes.

Responsibilities

What You Will Do:

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications
