CareRing Health in Maryland is seeking a compassionate LPN to join our home-based care team. You will conduct patient home visits, administer medications, and collaborate with physicians, nurses, and social workers to coordinate care for patients in their homes.

Responsibilities include assessing patient needs, planning and directing care, and documenting progress while complying with regulations and company policies.

Qualifications

• Active LPN license in Maryland.
• Valid driver’s license and reliable transportation.
• Minimum 2 years of nursing experience.

Responsibilities

What You Will Do:

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes - conduct patient home visits


• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs


• Manage emergency situations in a prompt, precise, and professional manner


• Complete medication administration and treatment needs for all assigned patients


• Provide nursing care in accordance with applicable regulations and Company policy


• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

Qualifications:

• Active LPN state license


• Valid drivers’ license


• Two or more years of applicable nursing experience
