Job summary

CareRing Health provides in-home nursing services and seeks compassionate LPNs to join our team delivering care to patients in their homes. You will collaborate with physicians, nurse practitioners, therapists, social workers, and caregivers to deliver coordinated home visits with emphasis on patient-centered planning and timely care.

Responsibilities

• Deliver comprehensive home-based care by collaborating with physicians, nurses, therapists, social workers and caregivers to provide coordinated care during patient home visits
• Assess, plan, direct, and evaluate each patients care plan to meet needs
• Manage emergency situations promptly and professionally
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with regulations and Company policy
• Prepare and maintain accurate clinical and progress notes in a timely basis

What You Will Do:

• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis
