Job summary

Amedisys in Portland, OR is seeking a licensed practical nurse for a full-time float role. Work Monday–Friday, 8am–5pm, providing home health and hospice services. Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to assigned state(s).
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify changes promptly.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
