Brooksby Village by Erickson Senior Living in Peabody, MA is seeking an LPN/LVN to provide compassionate, skilled nursing care to residents in a continuing care retirement community. The role emphasizes resident-first care, clinical excellence, and collaboration with the nursing team.

Responsibilities include administering medications, updating care plans, documenting in the EMR, supervising care staff, and participating in care meetings to improve resident outcomes.

Qualifications

• Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) certification valid in the state.
• CPR certification required.
• Minimum of 1 year of experience in assisted living, home health, or long-term care preferred.
• Experience with electronic medical records (EMR) is preferred.

Responsibilities

• Provide clinical care to residents according to nursing standards and Erickson Senior Living policies.
• Assess, observe, and document residents’ condition in the EMR; notify provider as needed.
• Update or modify care plans to reflect changes in care needs.
• Supervise and lead care associates.
• Administer medications timely and accurately according to orders.
• Participate in meetings related to resident outcomes and care.
• Respond promptly to residents and families with professional communication.
• Support RN in clinical reviews and quality indicators.
• Perform other duties as assigned by the RN clinical leader.

Location: Brooksby Village by Erickson Senior Living

Join our team as a Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN). The LPN/LVN is responsible for providing clinical care to residents in the neighborhood according to the nursing standards of practice, board of nursing, and Erickson Senior Living standards.

• Compensation: commensurate with experience, NEW RATES-$36 -$43 an hour plus shift differentials- $3.50-eves $4.00-nights and $4.50 for all shifts on the weekend!

How you will make an impact

• Developing relationships with the residents and their families using a consistent caregiver approach supporting the resident’s physical, spiritual, emotional, and clinical needs and focusing on placing the resident first.
• Assessing, observing, and evaluating resident’s clinical condition, document findings in the resident electronic medical record, and notify the provider and/or RN when necessary.
• Updating or modifying care plans as needed to reflect any changes to a resident’s care and service needs.
• Supervising and leading care associates
• Administering medications timely, accurately, and according to orders.
• Participating in meetings related to resident outcomes and care
• Responding to residents, family, or responsible parties in a timely and professional manner
• Participating in the review of clinical indicator reviews – falls, wounds, nutrition, antipsychotic use, etc.
• Other duties as assigned by RN clinical leader

What you will need

• Minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.
• Licensed by State as a Licensed Practical Nurse.
• CPR certification required.
• A passion to care for seniors and be a strong resident advocate.
• Experience with electronic medical records is preferred.
