Responsibilities

• Provide high-quality, resident-centered care including administering medications, wound care, and treatments.
• Monitor and report changes in resident condition with accurate documentation in EMR.
• Develop and implement individualized care plans with the interdisciplinary team.
• Participate in ongoing quality improvement initiatives to improve resident outcomes.
• Build and maintain relationships with residents, families, and caregivers, providing support and education.
• Maintain accurate records and ensure regulatory compliance.

12hrs. +Rotating Weekends

PRN/Per Diem. Full-Time Day. Part-Time & Full-Time Night

Responsibilities:

• Provide high-quality, resident-centered care to our seniors, including administering medications, wound care, and treatments as needed

• Monitor and report changes in resident condition, providing accurate and thorough documentation in our electronic medical records

• Develop and implement individualized care plans in collaboration with the interdisciplinary team, ensuring our residents' needs are met and exceeded
