Job summary

Affirmed Home Care, Inc. in Jersey City, NJ is seeking a compassionate Licensed Practical Nurse (LPN) to provide one-on-one home health care for a client in Jersey City. You’ll administer medications, monitor health status, and coordinate with family and care team to support comfort at home.

The role offers flexible scheduling (full-time, part-time, and per diem), competitive pay at $45 per hour, and benefits with ongoing training and support to grow professionally.

Qualifications

• Active New Jersey LPN license.
• 1+ year of nursing experience preferred.
• Current CPR/BLS certification.
• Strong clinical, communication, and organizational skills.
• Reliable, compassionate, and professional.
• Comfortable working independently in a home care setting.
• Valid driver’s license and reliable transportation preferred.
• EHR/documentation experience preferred.

Responsibilities

• Provide compassionate nursing care according to the client’s individualized care plan.
• Administer medications and prescribed treatments.
• Monitor the client’s condition and promptly communicate any changes.
• Provide wound, catheter, and other skilled nursing care within your scope of practice.
• Maintain accurate and timely clinical documentation.
• Promote the client’s comfort, safety, dignity, and independence.
• Communicate professionally with the client, family members, and care team.
