Job summary

Affirmed Home Care is seeking a compassionate Licensed Practical Nurse (LPN) to provide one-on-one care in New Brunswick and surrounding Central New Jersey areas. Qualifications

• Active LPN license in New Jersey is required.
• Minimum 1 year of nursing experience preferred.
• CPR/BLS certification is required.
• Strong communication and clinical skills are essential.
• Reliable, compassionate, and professional demeanor for home care.

Responsibilities

• Provide nursing care per individualized care plan.
• Administer medications and treatments as prescribed.
• Monitor client condition and report changes.
• Perform wound care, catheter care, and other skilled tasks within scope.
• Maintain accurate documentation in patient records.
• Support client safety, comfort, dignity, and independence.
• Communicate professionally with client, family, and care team.
