Job summary

JGS Lifecare's Leavitt Family Jewish Home in Massachusetts is seeking a licensed Practical Nurse (LPN) to deliver direct resident care, administer medications, monitor vital signs, and follow physician orders and care plans.

The role requires active MA LPN licensure, strong clinical skills in nursing procedures, safety and infection control, and excellent communication in English. We offer competitive hourly pay, shift differentials, and a supportive team.

Qualifications

• Active, unencumbered MA LPN licensure.
• Strong clinical skills in medication administration, vital signs, and EHR documentation.
• Knowledge of long-term care regulations and patient safety protocols.
• Current CPR certifications (BLS and ACLS).
• Ability to communicate fluently in English.

Responsibilities

• Deliver direct resident care including medications, vital signs, and prescribed treatments.
• Assist residents with ADLs such as bathing, dressing, grooming, feeding, mobility, and toileting.
• Monitor and document clinical status, wound care progress, intake/output, and treatment responses.
• Collaborate with interdisciplinary teams to implement care plans and ensure continuity of care.
• Maintain infection control compliance and a safe environment for residents and staff.

$35.00 - $39.00/hour, based on experience Shift Differentials: $2.50/HR evenings, $3.50/HR overnights. Up to $5.00/HR on weekends

Responsibilities

• Delivering Solutions that Matter: Provide direct resident care including administering medications, monitoring vital signs, and performing prescribed treatments in accordance with physician orders and nursing care plans.
• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, feeding, mobility support, and toileting while promoting dignity and independence.
• Monitor and document residents' clinical status, including changes in condition, wound care progress, intake/output, and responses to treatments, reporting significant changes.
• Collaborate with interdisciplinary care teams to implement and update individualized care plans and ensure continuity of care.
• Ensure compliance with infection control protocols, safety regulations, and facility policies while maintaining a clean, safe environment for residents and staff.

Qualifications for Success

• Active, unencumbered MA Licensed Practical Nurse (LPN) licensure
• Strong clinical skills in basic nursing procedures, including medication administration, vital signs monitoring, and documentation in electronic health records (EHR).
• Knowledge of long-term care regulations, infection control standards, and patient safety protocols in a skilled nursing or similar healthcare setting.
• Current CPR certifications, including BLS and ACLS.
• Ability to communicate fluently in English - both verbally and written
