Chelsea Jewish Lifecare seeks a Licensed Practical Nurse (LPN) to join the Brudnick Center for Living. The role involves direct patient care, medication administration, monitoring vital signs, and adherence to care plans in a long-term care environment.

You will collaborate with interdisciplinary teams and ensure infection control and safety standards are maintained in a resident-focused setting. English fluency is required and MA LPN licensure is mandatory.

Responsibilities

Licensed Practical Nurse – LPN | Brudnick Center for Living

$35.00 - $39.00/hour, based on experience. Full-Time | Part-Time | PRN.

Delivering Solutions that Matter

• Provide direct resident care including administering medications, monitoring vital signs, and performing prescribed treatments in accordance with physician orders and nursing care plans.
• Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, feeding, mobility support, and toileting while promoting dignity and independence.
• Monitor and document residents' clinical status, including changes in condition, wound care progress, intake/output, and responses to treatments, reporting significant changes.
• Collaborate with interdisciplinary care teams to implement and update individualized care plans and ensure continuity of care.
• Ensure compliance with infection control protocols, safety regulations, and facility policies while maintaining a clean, safe environment for residents and staff.

Qualifications for Success

• Active, unencumbered MA Licensed Practical Nurse (LPN) licensure.
• Strong clinical skills in basic nursing procedures, including medication administration, vital signs monitoring, and documentation in electronic health records (EHR).
• Knowledge of long-term care regulations, infection control standards, and patient safety protocols in a skilled nursing or similar healthcare setting.
• Current CPR certifications, including BLS and ACLS.
• Ability to communicate fluently in English – both verbally and written.
