BrightStar Care of Concord, Lexington and Woburn in Lexington, MA is seeking a Licensed Practical Nurse (LPN) to deliver compassionate home health care in clients' homes. You will join a supportive team focused on enhancing patients' lives across the region.

Responsibilities include medication administration, vital signs monitoring, daily living assistance, patient education, and care coordination with RNs and physicians. Travel to patient homes is required.

Qualifications

• Active and valid LPN license in Massachusetts.
• Minimum 1 year of experience in home health or related healthcare setting.
• Current BLS certification.
• Reliable transportation and willingness to travel to patient homes.
• Proficient in electronic health records (EHR).
• Excellent communication and interpersonal skills.
• Strong sense of compassion and dedication to quality patient care.
