Job summary

BrightStar Care of Concord, Lexington, and Woburn is seeking a Licensed Practical Nurse (LPN) to provide compassionate home health care to clients in their homes across the region. You will join a supportive, high-performing team and follow care plans, document in EHR, and coordinate with RNs, therapists, and physicians.

This role offers flexible scheduling and competitive hourly pay. A valid MA LPN license and BLS certification are required, along with at least one year of home health

Qualifications

• Active and valid LPN license in Massachusetts.
• Minimum 1 year of experience in home health or related healthcare setting.
• Current BLS certification.
• Reliable transportation and willingness to travel to patient homes.
• Proficiency in electronic health records (EHR).
• Strong communication and interpersonal skills.
• Compassion and commitment to quality patient care.

Responsibilities

• Administer medications and treatments as prescribed by healthcare providers.
• Monitor and document patient vital signs and overall health status.
• Assist with daily living activities such as bathing, grooming, and meal preparation.
• Educate patients and families on managing health conditions and care plans.
• Coordinate with RNs, therapists, physicians, and other healthcare professionals.
• Maintain accurate and timely documentation using EHR systems.
• Provide emotional support and companionship.
• Follow all safety and health regulations in home care settings.
