BrightStar Care of Concord, Lexington and Woburn is seeking a Licensed Practical Nurse (LPN) to provide compassionate home health care to clients in their homes across the region. You will administer medications, monitor vital signs, assist with activities of daily living, and coordinate with RNs, therapists, and physicians to ensure quality care.

MA LPN license and BLS are required, with at least 1 year in home health.

Formación

• MA LPN license active in MA.
• Minimum 1 year in home health or related setting.
• Current BLS certification required.

Responsabilidades

• Administer medications and treatments per provider orders.
• Monitor and document patient vital signs and status.
• Assist with activities of daily living (ADLs).
• Educate patients and families on care plans.
• Coordinate with RNs, therapists, physicians and other staff.
• Maintain timely documentation using EHR systems.
• Provide emotional support and companionship.
• Follow all home care safety and health regulations.

Licensed Practical Nurse (LPN) - Home Health

Salary:

$35-$45 per hour

Bonuses:

Responsibilities:

• Administer medications and treatments as prescribed by healthcare providers
• Monitor and document patient vital signs and overall health status
• Assist with daily living activities such as bathing, grooming, and meal preparation
• Educate patients and families on managing health conditions and care plans
• Coordinate with RNs, therapists, physicians, and other healthcare professionals
• Maintain accurate and timely documentation using EHR systems
• Provide emotional support and companionship
• Follow all safety and health regulations in home care settings

Requirements:

• Active and valid LPN license in Massachusetts
• Minimum 1 year of experience in home health or related healthcare setting
• Current BLS certification
• Reliable transportation and willingness to travel to patient homes
• Proficiency in electronic health records (EHR)
• Strong communication and interpersonal skills
• Compassion and commitment to quality patient care
