Job summary

Ohana Pacific Health is seeking a Licensed Practical Nurse to support wound care and resident-centered care at Hale Makua Health Services in Maui. You will follow facility policies to deliver professional nursing care, administer meds, and coordinate with the care team.

Qualifications

• Graduate of an accredited school of nursing.
• Current Hawaii LPN license.
• Knowledge of medical terminology and abbreviations.
• Experience in geriatrics preferred.
• Current and valid 2 Step TB clearance and health review.
• First Aid and CPR preferred.

Responsibilities

• Provide professional nursing care to residents following established policies and procedures.
• Administer medications and treatments as ordered by the physician.
• Assess patient status and communicate changes to the care team.
• Collaborate with physicians, therapy services, and agencies to improve care quality.

The Position

We're looking for passionate Licensed Practical Nurses to help us with providing professional nursing care to residents following established policies and procedures of the facility.

• Demonstrates knowledge of the patient's diagnoses, limitations, expectations, plan of health, and current status.
• Administers medications and treatments as ordered by the physician and according to policy and procedures.
• Appraises the family, physician and others of changes/incidents.
• Collaborates with physicians, therapy services, consultants and agencies to improve quality of services and to resolve identified problems.

Requirements

• Graduate of an accredited school of nursing.
• Current license as a License Practical Nurse in the State of Hawaii.
• Knowledge of medical terminology and abbreviation.
• Experience in geriatrics and current nursing standards of practice preferred.
• Current and valid 2 Step TB clearance and health review (can be done in-house).
• First Aid and CPR preferred (can be done in-house).
