Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in clients’ homes under the supervision of a registered nurse. You’ll coordinate with the interdisciplinary team, patients, families, and referring agencies to ensure comprehensive care.

The role emphasizes professional development with access to training and growth opportunities within a growing national provider. Strong patient care and home health experience are preferred.

Formación

• Graduate of an accredited school of vocational nursing.
• Current state license as an LPN with current driver's license.
• Two years’ experience as an LPN; home health experience preferred.
• Proof of current CPR and Hepatitis profile.

Responsabilidades

• Administer skilled nursing care under RN supervision in clients' homes.
• Coordinate care with the interdisciplinary team, patient/family and referring agency.

Educación

Graduate of an accredited school of vocational nursing

Current state license as an LPN

Driver's License

Two years’ experience as an LPN, home health preferred

About the position:

Primary function of the LPN is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency.

Qualifications:

• Graduate of an accredited school of vocational nursing.
• Current state license as an LPN, Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile
