Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care in clients' homes under RN supervision. You will collaborate with an interdisciplinary team, patients and families to coordinate comprehensive care.

The position requires an LPN license, a current CPR and hepatitis profile, and a valid driver's license. Home health experience is preferred, with opportunities for professional development and advancement.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current LPN license in good standing.
• Driver’s license is required.
• Two years’ home health experience preferred.
• CPR certification and Hepatitis profile current.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse in clients’ homes.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.

Education

About the position:

The primary function of the LPN is to administer skilled nursing care, under the supervision of a registered nurse, for clients of all ages in their place of residence. Coordinates care with the interdisciplinary team, patient/family, and referring agency.

Qualifications:

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.
