Seven Hills Pediatric Center is seeking an LPN for our Medically Complex Behavioral Residence in Groton, MA. You will provide individualized nursing care, health coordination, and advocacy for children and youths with complex medical and behavioral needs in a trauma-informed, supportive environment.

Qualifications

• Licensed Practical Nurse (LPN) with MA license.
• Experience providing care to children with complex medical/behavioral needs.
• Trauma-informed care approach and professional nursing judgment.
• Ability to document nursing interventions accurately.
• Strong collaboration with families, medical providers and staff.

Responsibilities

• Provide individualized nursing and medical support to children and youth with complex needs.
• Assess health needs and coordinate medical follow-up and care plans.
• Monitor medications and treatment orders; report adverse reactions.
• Document interventions, health changes and appointments in a timely manner.
• Assist with medical appointments and transportation as directed.
• Maintain plans of care and health records per regulatory requirements.
• Communicate concerns and plans to residential and school teams.
• Mentor Direct Support Professionals and supervise staff as needed.
• Support training on medication policies and healthcare practices.

Schedule: Nights, 11pm-7pm; Every other weekend & holidays

What You’ll Do

• Provide individualized nursing and medical support to children and youth with complex medical and behavioral needs.
• Assess participants’ health needs and coordinate appropriate medical care and follow-up.
• Monitor and maintain current medication and treatment orders, including identifying and reporting potential side effects or adverse reactions.
• Document nursing interventions, health changes, treatments, appointments, and other required information accurately and timely.
• Assist participants with medical appointments and transportation as directed.
• Review and maintain plans of care and health records in accordance with regulatory and program requirements.
• Communicate medical concerns, recommendations, treatment plans, and upcoming appointments to residential and school-based teams.
• Serve as a nursing resource and mentor to Direct Support Professionals, supervisors, and other program staff.
• Assist with training staff on medication policies, procedures, and other healthcare-related practices.
• Provide direction and supervision to direct care staff to promote safe and appropriate medical care.
• Participate in team meetings and collaborate with medical, behavioral health, educational, and residential professionals.
• Ensure participants are appropriately observed and supported to maintain their safety and optimal health.
• Maintain confidentiality and uphold the dignity, rights, and individuality of every person receiving services.
• Complete required training and participate in educational opportunities to maintain and enhance professional knowledge.
• Respond appropriately to medical or behavioral emergencies and perform additional duties as assigned.
