Seven Hills Foundation is seeking an LPN Supervisor to lead the nursing team and provide direct care within designated programs. The role emphasizes oversight, compliance with Nurse Practice Act, and coordination of healthcare services.

This leadership position also supports supervision and training of nursing staff, contributes to clinical decisions, and ensures medication protocols are followed and documented in EHR systems.

Formación

• Current LPN license in good standing.
• Experience supervising nursing staff and coordinating care.
• Knowledge of Nurse Practice Act and state regulations (DDS).
• Proficient with medication administration and MAP protocols.

Responsabilidades

• Provide high-quality nursing care and medication administration per orders.
• Serve as clinical resource and mentor to LPNs and direct care staff.
• Oversee onboarding, orientation, and training of nursing staff and competencies.
• Monitor health care plans and treatment protocols for assigned programs.
• Assist scheduling and coordination of nursing coverage across programs.
• Communicate medical issues to RNs, Health Care Directors, and leadership.
• Ensure documentation in EHR is complete and compliant.
• Support attendance at medical appointments and follow-up care.
• Participate in MAP training and ensure MAP standards are followed.

Pay: $42.00-$44.00/hour

Schedule: Week 1: Sunday 3pm-11pm. Monday 3pm-11pm, Tuesday 9am-5pm, Wednesday 9am-5pm, Thursday 9am-5pm; Week 2: Monday 3pm-11pm, Tuesday 9am-5pm, Wednesday 9am-5pm, Thursday 3pm-11pm, Saturday 3pm-11pm; Mandatory staff meeting every 3rd Thursday of the month 1pm-3pm.

The Licensed Practical Nurse (LPN) Supervisor plays a critical leadership role within the nursing team, providing direct care to individuals while supporting the oversight and coordination of healthcare services across designated programs. The LPN Supervisor ensures high-quality medical support in accordance with healthcare regulations, organizational standards, and the Nurse Practice Act. This position also supports supervision and training of other nursing staff, contributes to clinical decision-making, and ensures compliance with all medication and treatment protocols.

What You'll Do

• Provide high-quality nursing care and medication administration to individuals in accordance with physicians' orders and agency protocols
• Serve as a clinical resource and mentor to other LPNs and direct care staff within assigned programs
• Support the onboarding, orientation, and training of nursing staff, including oversight of clinical competencies
• Monitor the implementation of health care plans and treatment protocols
• Assist in the scheduling and coordination of nursing coverage across programs, ensuring consistent and adequate staffing
• Communicate medical issues promptly and effectively to RNs, Health Care Directors, physicians, and program leadership
• Conduct or assist in health assessments and ensure timely documentation in Electronic Health Records (EHR)
• Accompany individuals to medical appointments when necessary, and ensure follow-up care is completed and documented
• Monitor and maintain compliance with Medication Administration Program (MAP) standards and provide MAP training as required
• Participate in incident reporting, follow-up, and resolution in collaboration with clinical and administrative leadership
• Ensure health records and documentation are complete, accurate, and compliant with DDS and agency standards
• Promote a culture of respect, safety, and dignity in all interactions with individuals and staff
• Perform other nursing or administrative duties as assigned by the supervising RN or Health Care Director
