Job summary

Angels Care Home Health is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care in clients' homes under the supervision of a registered nurse. You will coordinate care with the interdisciplinary team and referring agencies to ensure high-quality outcomes.

Ideal candidates have an accredited LPN program graduate, current Nebraska LPN license, CPR, Hepatitis profile, and at least two years of home health experience, plus a valid driver's license.

Qualifications

• Graduate of an accredited school of vocational nursing.
• Current state license as an LPN, Driver's license.
• Two years’ experience as an LPN, home health preferred.
• Proof of CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under RN supervision in clients' homes.
• Coordinate care with interdisciplinary team, patient/family and referring agency.
• Document care plans and maintain accurate records.
