Responsibilities

The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs technical skilled care in the patient's home.

• Provides services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team. May not alter the plan of care.

• Instructs the patient's family on care and maintaining a healthy environment for the patient.

• Observes, records, and reports the patient's response to treatment and any changes in condition to the supervising nurse or physician daily.

Education and Experience

License Requirements:

• Current LPN licensure in the state of practice
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or reliable transportation
• CPR certification required
