Job summary

CareRing Health, LLC is seeking compassionate LPNs to provide home-based nursing care across our patient homes in Maryland. You will work with physicians, nurses, therapists, social workers, and caregivers to deliver coordinated care during home visits, ensuring patient comfort and safety.

Qualifications

• Active LPN state license.
• Valid driver’s license.
• Two or more years of applicable nursing experience.

Responsibilities

• Deliver comprehensive home-based nursing care with a multidisciplinary team.
• Assess and adjust each patient’s care plan to meet needs.
• Manage emergency situations promptly and professionally.
• Complete medication administration and treatment for assigned patients.
• Maintain timely, accurate clinical and progress notes.

Responsibilities

What You Will Do:

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

Qualifications:

• Active LPN state license
• Valid drivers’ license
• Two or more years of applicable nursing experience
