Job summary

Charlestown by Erickson Senior Living is seeking Licensed Practical Nurses to join our continuing care team. Full-time Day 7:00am–3:30pm and Part-time Night 11:00pm–7:00am positions are available in a vibrant senior living community.

You will provide compassionate, resident‑centered care, assess conditions, update care plans, administer medications, and supervise care associates. A minimum of 1 year of LPN experience is required; experience in assisted living or long‑term care is preferred; EMR

Qualifications

• Minimum of 1 year of experience as an LPN.
• Experience in assisted living, home health, or long-term care is preferred.
• Experience with electronic medical records (EMR) is preferred.

Responsibilities

• Develop relationships with residents and families using a consistent caregiver approach.
• Assess, observe, and document residents' clinical condition and notify RN/provider when needed.
• Update or modify care plans to reflect changes in resident needs.
• Supervise and lead care associates.
• Administer medications accurately and timely.
• Participate in meetings related to resident outcomes and care.
• Respond to residents and families professionally and promptly.
• Participate in clinical indicator reviews (falls, wounds, nutrition, etc.).
• Other duties as assigned by the RN clinical leader.

We are seeking Licensed Practical Nurses to join our continuing care team here at Charlestown!

Positions Available

• Full Time Day Shift 7:00am - 3:30pm
• Part Time Night Shift 11:00pm - 7:00am

Compensation

Ranging from $32.00 per hour to 39.00 per hour; based upon years of licensure. Potential for $2.00 per hour shift differential for evening and night shifts.

How You Will Make An Impact

• Developing relationships with the residents and their families using a consistent caregiver approach supporting the resident’s physical, spiritual, emotional, and clinical needs and focusing on placing the resident first.
• Assessing, observing, and evaluating resident’s clinical condition, document findings in the resident electronic medical record, and notify the provider and/or RN when necessary.
• Updating or modifying care plans as needed to reflect any changes to a resident’s care and service needs.
• Supervising and leading care associates
• Administering medications timely, accurately, and according to orders.
• Participating in meetings related to resident outcomes and care
• Responding to residents, family, or responsible parties in a timely and professional manner
• Participating in the review of clinical indicator reviews - falls, wounds, nutrition, antipsychotic use, etc.
• Other duties as assigned by RN clinical leader

What You Will Need

• A minimum of 1 year of experience working as an LPN required.
• Experience in assisted living, home health, or long-term care is preferred.
• A passion to care for seniors and be a strong resident advocate.
• Experience with electronic medical records is preferred.
