Job summary

Sunrise Community, Inc. is seeking a Licensed Practical Nurse to provide basic medical services for individuals with intellectual and developmental disabilities on-site in Bartow, Florida.

The role involves performing routine health checks, documenting accurately, and coordinating with Direct Support Professionals to meet each person’s needs. This position requires a valid Practical Nurse license, a valid driver’s license, and basic computer skills.

Qualifications

• Basic computer skills required.
• Must be licensed Practical Nurse.
• Experience with individuals with intellectual/developmental disabilities preferred.

Responsibilities

• Assist in providing basic healthcare services per individual care plan.
• Perform tasks such as blood pressure checks, weights, and other delegated duties.
• Ensure documentation is accurate and complete per regulations.
• Maintain organization and cleanliness of supplies and equipment; restock as needed.
• Collaborate with Direct Support Professionals to meet individuals’ needs.
• Report any changes in medical or behavioral status to supervisor.
