Job summary

We are seeking a Licensed Practical Nurse to implement basic healthcare services, monitor vital signs, and document accurately. The role requires a valid LPN license, a clean driving record, and basic computer skills.

Qualifications

• Must successfully complete the required pre-employment background and drug screenings.
• Must possess a valid driver’s license with endorsements and a minimum of three years of driving experience.
• Must possess a current and valid license for Practical Nurse.
• Experience working with people with intellectual/developmental disabilities is preferred.

Responsibilities

• Assist in providing basic healthcare services according to the individual’s plan, ensuring optimal health and wellbeing.
• Monitor blood pressure, weight, and other assigned tasks as delegated.
• Ensure all documentation is accurate and complete according to regulations.
• Organize and maintain the cleanliness of supplies and equipment, restocking inventory as needed.
• Collaborate with Direct Support Professionals to meet the individuals’ needs.
• Communicate immediately any changes in medical or behavioral status to the supervisor.
• Perform all other duties as assigned.

Compensation: Base hourly rate or salary is determined by geographic location, skills, education and relevant experience.
