• Collaborates with the clinical team to conduct patient assessments (OASIS) and adjust plans as needed.
• Educates patients and family on exercise programs, safety, and self-care activities.
• Maintains accurate patient records and ensures clean, safe treatment areas.
• Delivers PT treatments within productivity standards and participates in care planning meetings.
