Job summary

You will work with physicians and the rehabilitation team to design coordinated treatment plans, adjust as needed, document progress, and prepare patients for discharge with home exercise programs, while meeting productivity expectations.

Qualifications

• Arizona licensed PT required.
• Bachelor’s degree in Physical Therapy.
• BLS certification required.

Responsibilities

• Assess and plan treatment per patient needs.
• Review patient history and physician referral.
• Develop and adjust treatment programs with the care team.
• Document evaluations, progress, and discharge plans.
• Communicate with patients, families, and staff about progress and home programs.
