Job summary

Paycom - ATS is seeking a licensed Physical Therapist in Arizona to participate in evaluation, planning, recording, and implementation of patient care programs designed to maintain, improve, or restore physical function. The role includes collaborating with physicians and the rehab team, adjusting treatment plans, and documenting progress and discharge instructions.

A Bachelor’s degree in Physical Therapy and Arizona licensure are required, along with hands-on BLS.

Qualifications

• Licensed in the state of Arizona
• Bachelor’s Degree in Physical Therapy required
• BLS certification required or current

Responsibilities

• Assess and plan of care based on patient history and referrals.
• Directs and implements PT interventions to maintain, improve, or restore function.
• Re-evaluates patients and adjusts treatment plans as needed in collaboration with the care team.
• Documents evaluations, progress notes, discharge planning, and patient education.
• Maintains communication with patients, families, and staff regarding goals and progress.
• Coordinates patient care and discharge planning in partnership with physicians and family.
