Brooks Rehabilitation in Phoenix, AZ seeks a Physical Therapist under general supervision to participate in evaluation, planning and implementation of patient care programs to restore function. Hours are 40 per week with a comprehensive benefits package.

Responsibilities include reviewing history, developing treatment plans, coordinating with physicians, documenting progress and preparing patients for discharge with home programs. Excellent communication and productivity are essential.

Responsabilidades

• Assessment and Plan of Care/Treatment
• Reviews patient history and interprets physician referral
• Evaluates patient's status and physical therapy problems and abilities using objective and subjective techniques
• Develops and adjusts treatment program based on patient/family needs
• Consults with physicians, other rehabilitation team members, patient and family to design a coordinated treatment plan
• Performs patient treatment through the use of physical therapy interventions
• Re-evaluates patient's status and environmental factors to modify/terminate the program as appropriate
• Maintains active communication with patients and others regarding progress, home programs and related issues
• Prepares patient for discharge with home programs and caregiver instruction
• Manages schedule to meet department productivity expectations
• Attends staff meetings and in-services
• Documents evaluations, progress notes, discharge recommendations and education forms
