Job summary

Salem Health & Rehabilitation Center in Salem, VA is seeking a caring Physical Therapist licensed in Virginia or eligible for licensure, to join our experienced rehabilitation team. You will help patients regain strength, skills, independence and confidence to return home and resume an active life.

The PT evaluates residents, develops treatment plans, obtains physician approval, contributes to discharge planning, and documents care while supervising aides and PTAs.

Qualifications

• Virginia Physical Therapy License or eligibility to obtain licensure in Virginia.
• Excellent verbal and written communication abilities.
• Strong problem-solving and clinical reasoning skills.

Responsibilities

• Evaluate residents and develop treatment plans.
• Obtain approval for services from the referring physician and document care.
• Discharge planning, records of evaluation and treatment, charges.
• Supervise Physical Therapist Assistants and Aides; screen residents for PT needs.
