Job summary

Camellia Gardens Center for Nursing & Rehabilitation in Henderson, NC is seeking a licensed Physical Therapist (PT) to join our rehabilitation department and support residents in improving mobility and independence. You will evaluate residents, develop individualized treatment plans, provide therapeutic interventions, document care, and collaborate with nursing staff and physicians to optimize outcomes.

Qualifications

• Must be a graduate of an accredited Physical Therapy program.
• Current and active Physical Therapist license in North Carolina.
• Strong clinical assessment and problem-solving abilities.
• Excellent communication and resident-centered care.

Responsibilities

• Perform physical therapy evaluations to identify mobility limitations and goals.
• Develop individualized treatment programs based on assessments and physician orders.
• Provide interventions to improve strength, balance, coordination, and mobility.
• Monitor outcomes and update care plans as residents progress.
• Maintain complete documentation in accordance with standards.
• Collaborate with nursing staff, physicians, and other rehab professionals.
• Instruct residents, families, and caregivers on mobility techniques and safety.
• Supervise PT Assistants and other therapy personnel.
• Participate in interdisciplinary meetings, discharge planning, and facility improvements.
• Support staff education and training related to safe mobility and restorative care.
