Job summary

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and active Physical Therapist license in North Carolina.
• Strong clinical assessment, treatment planning, and problem-solving abilities.
• Excellent communication skills and a resident-centered approach to care.
• Ability to work independently while contributing to a collaborative healthcare team.
• Ability to lift up to 50 pounds and assist residents with mobility and transfers, with or without reasonable accommodation.
• Ability to stand, walk, bend, push, and pull throughout the workday.

Responsibilities

• Perform physical therapy evaluations to identify mobility limitations, functional needs, and rehabilitation goals.
• Develop individualized treatment programs based on clinical assessments and physician orders.
• Provide therapeutic interventions to improve strength, balance, coordination, endurance, and mobility.
• Monitor treatment outcomes and update care plans as residents progress.
• Maintain complete and accurate clinical documentation in accordance with professional and regulatory standards.
• Work closely with nursing staff, physicians, and other rehabilitation professionals to coordinate resident care.
• Provide instruction to residents, families, and caregivers on mobility techniques, safety, and restorative exercises.
• Oversee and provide guidance to Physical Therapist Assistants and other assigned therapy personnel.
• Participate in interdisciplinary meetings, discharge planning, and facility improvement initiatives.
• Support staff education and training related to safe mobility and restorative care practices.
