Job summary

Camellia Gardens Center for Nursing & Rehabilitation in Henderson, NC seeks a dedicated Occupational Therapist (OT) to join our full-time rehabilitation team. You will assess residents, set individualized goals, and deliver therapies to improve daily living skills and independence.

You'll collaborate with an interdisciplinary team, document progress, and contribute to discharge planning while maintaining compliance with Medicare guidelines and safety standards.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current North Carolina Occupational Therapist license.
• Experience in therapeutic assessments and goal‑directed plans.
• Strong communication and organizational skills.
• Ability to work with interdisciplinary teams.
• Experience in skilled nursing or long‑term care preferred.
• Ability to assist residents with activities and lift up to 50 pounds.
• Willingness to travel locally for coverage.

Responsibilities

• Conduct comprehensive OT evaluations to identify needs.
• Establish individualized care plans based on assessments.
• Provide interventions focused on ADLs, coordination, motor skills, strength and independence.
• Review progress and adjust treatment with the rehab team.
• Supervise therapy assistants and students.
• Maintain accurate evaluations, notes and discharge documentation.
• Educate residents and families on adaptive techniques and safety.
• Participate in interdisciplinary conferences and discharge planning.
• Follow Medicare guidelines and documentation requirements.
• Contribute to quality improvement and staff education.
• Maintain equipment and safe treatment areas.
• Provide coverage at nearby facilities; travel reimbursement provided.
