Park Avenue Nursing Home in Meadville, PA seeks a flexible PRN Physical Therapist Assistant to join our rehabilitation team. Provide skilled treatments under a licensed PT and support resident mobility and independence.

You’ll work with nursing and therapy staff, educate residents and caregivers, and document sessions in Net Health. Prior SNF/LTC experience and PA PTA license preferred.

Formación

• Graduate of an accredited Physical Therapist Assistant (PTA) program.
• Active Pennsylvania PTA license in good standing required.
• Pennsylvania Indirect Supervision License preferred.
• SNF/LTC experience preferred.
• Experience with Net Health preferred, but training can be provided.

Responsabilidades

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Monitor resident progress and communicate changes to the supervising Physical Therapist.
• Collaborate with nursing staff and other healthcare professionals to provide exceptional resident care.
• Educate residents and caregivers on therapeutic exercises, mobility, and safety techniques.
• Maintain accurate and timely clinical documentation using Net Health.
