Park Avenue Nursing Home in Meadville, PA is seeking a PRN Physical Therapist Assistant to join our rehabilitation team. The PTA will provide skilled treatments under the supervision of a licensed Physical Therapist to help residents regain mobility and independence in a SNF/LTC setting.

This flexible PRN position offers opportunities to collaborate with a supportive rehabilitation team, document care using Net Health, and contribute to resident quality of life.

Competenze

• Graduate of an accredited Physical Therapist Assistant (PTA) program.
• Active Pennsylvania PTA license in good standing required.
• Pennsylvania Indirect Supervision License preferred.
• SNF/LTC experience preferred.
• Experience with Net Health preferred, but training can be provided.
• Strong communication, organizational, and interpersonal skills.
• Compassionate, dependable, and committed to providing quality resident care.

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Monitor resident progress and communicate changes to the supervising Physical Therapist.
• Collaborate with nursing staff and other healthcare professionals to provide exceptional resident care.
• Educate residents and caregivers on therapeutic exercises, mobility, and safety techniques.
• Maintain accurate and timely clinical documentation using Net Health.
• Help create a positive, encouraging rehabilitation experience for every resident.
