Qualifikationen

• Graduate of an accredited Physical Therapy program.
• Current, active Physical Therapist license in Pennsylvania.
• SNF/LTC experience preferred, but not required.
• Experience with Net Health and PCC preferred; training can be provided.
• Strong communication, clinical, and organizational skills.
• A positive attitude and genuine passion for helping others.
• Ability to work independently while contributing to a supportive team.

Aufgaben

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide skilled physical therapy services to residents in a SNF/LTC setting.
• Help residents improve strength, balance, mobility, and functional independence.
• Monitor progress and adjust treatment plans to support positive outcomes.
• Collaborate with PTAs, nursing staff, and the interdisciplinary care team.
• Educate residents, families, and caregivers on therapeutic exercises and safety.
• Maintain accurate and timely documentation using Net Health and PCC.
• Provide compassionate, high-quality care that enhances residents' quality of life.
