Job summary

Meadowview Rehabilitation & Nursing Center in Lafayette Hill, PA is seeking a Full-Time Physical Therapist Assistant (PTA) to join our in-house therapy team in a skilled nursing and rehabilitation environment. Under the supervision of a licensed PT, you will implement personalized treatment plans to improve mobility and independence for residents.

The ideal candidate holds a PA PTA license, completed an accredited program, and values teamwork, clear documentation in Net Health and PCC, and

Qualifications

• Current Physical Therapist Assistant license in Pennsylvania (PA)
• Graduate of an accredited Physical Therapist Assistant program
• SNF or Long-Term Care experience preferred but new graduates are encouraged to apply
• Experience with Net Health and PCC is a plus
• Excellent communication, organizational, and teamwork skills
• Passion for delivering high-quality, resident-centered care

Responsibilities

• Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
• Implement individualized treatment plans for short-term rehabilitation and long-term care residents.
• Assist patients in improving strength, balance, mobility, endurance, gait, and functional independence.
• Monitor resident progress and communicate updates to the supervising Physical Therapist.
• Educate residents and families on therapeutic exercises, mobility techniques, and fall prevention strategies.
• Complete accurate and timely documentation using Net Health and PCC.
• Collaborate with physicians, nursing staff, and the interdisciplinary care team to achieve the best possible patient outcomes.
• Maintain compliance with all federal, state, and facility regulations.
