Job summary

Gardens of Euclid Beach in Cleveland, OH seeks a Full-Time Physical Therapist Assistant (PTA) to join our in-house rehabilitation team. This role focuses on skilled PT services for a geriatric SNF/LTC population, under the guidance of licensed PTs.

You will collaborate with an interdisciplinary team and document care using Net Health, delivering resident-centered, compassionate therapy to improve mobility and independence.

Qualifications

• Current Ohio PTA license.
• Graduate of an accredited Physical Therapist Assistant program.
• Strong communication and teamwork skills.
• Compassionate, dependable, and resident-focused.
• SNF/LTC experience is preferred but not required.
• New graduates are welcome to apply!
• Experience with Net Health is a plus.

Responsibilities

• Provide skilled physical therapy treatments under the direction of a licensed Physical Therapist.
• Carry out individualized treatment plans designed to improve mobility, strength, balance, and functional independence.
• Work with a diverse geriatric population in a SNF/LTC setting.
• Collaborate with PTs, OTs, COTAs, SLPs, nursing, and other members of the interdisciplinary team.
• Monitor resident progress and communicate changes to the supervising PT.
• Complete accurate and timely clinical documentation using Net Health.
• Provide compassionate, resident-centered care.

Gardens of Euclid Beach is seeking a motivated and compassionate Full-Time Physical Therapist Assistant (PTA) to join our in-house rehabilitation team in Cleveland, Ohio!

What You’ll Do
