Job summary

Centra Healthcare is seeking an experienced or newly graduated Occupational Therapist to join our skilled nursing/ LTC team. This direct hire position offers the opportunity to apply high-quality OT services in collaboration with physicians and a dedicated care team.

You will assess patient needs, develop and adjust care plans, and document interventions while coordinating with other disciplines to achieve patient goals.

Qualifications

• Must have graduated from an accredited school.
• Current state license as an Occupational Therapist / OT or in the process of applying for licensure.
• Current CPR certification.

Responsibilities

• Provide skilled occupational therapy services / interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals of occupational therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
