Job summary

Centra Healthcare Solutions Inc. is seeking an Occupational Therapist (OT) for a SNF/LTC setting on a permanent, direct-hire basis.

Both experienced and newly graduated candidates are encouraged to apply, with a supportive team and ongoing professional growth. You will assess referred patients, establish and administer treatment plans, educate patients and families, document progress, and collaborate with all disciplines to meet OT goals under physician direction.

Qualifications

• Graduated from an accredited school.
• Current state license as an Occupational Therapist / OT, or actively pursuing licensure.
• Current CPR certification.

• Establish and administer a treatment program with specific goals based on patient capacity and tolerance under physician direction.
• Set realistic and achievable goals for patients, documenting and recording patient conditions.
• Educate patients and families on appropriate occupational therapy methods.
• Provide occupational therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate and modify care to meet occupational therapy goals.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.
