Job summary

Centra Healthcare is seeking an experienced or newly graduated Occupational Therapist to join their growing team for a perm/direct hire assignment in SNF/LTC settings. Must have graduated from an accredited school, hold or be pursuing OT licensure, and maintain CPR certification.

The role emphasizes providing skilled OT services per physician orders, assessing needs, planning and evaluating care, and collaborating with the care team, including Early Intervention involvement.

Qualifications

• Graduated from an accredited school.
• Current state license as OT or in the licensure process.
• Current CPR certification.
• Provide skilled occupational therapy services per physician orders.
• Assess patient needs, plan, evaluate and modify care for OT interventions.
• Collaborates with all disciplines to plan and evaluate team goals.
• Includes Early Intervention.
