Job summary

AdventHealth seeks a licensed Master Social Worker to provide home-based social work services across six counties, addressing physical, emotional and spiritual needs of patients and families. You'll assess needs, coordinate resources, and support age-spanning populations in a medical setting.

This part-time role involves travel to patients' homes with mileage reimbursement and provided devices. Strong English communication, community resource knowledge, and licensure are essential.

Qualifications

• Master's degree in Social Work required.
• 1+ year experience in social services
• Experience with adult or geriatric population
• Basic Life Support (CPR) certification required
• Driver's license required
• Auto Insurance required
• LMSW or LSW licensure required

Responsibilities

• Uses the social work process to address physical, emotional, psychosocial, and spiritual needs of patients and families across adolescent, adult, and geriatric populations.
• Reviews social and emotional factors affecting medical treatment and communicates with physicians and team members for direction.
• Conducts systematic assessments to develop individualized plans with goals and outcomes.
• Identifies resources and community agencies to assist patients in accessing appropriate care, provides crisis intervention and counseling, and documents updates.
• Implements and evaluates care plans and maintains knowledge of community resources and referrals.
• Other duties as assigned.
