Job summary

AdventHealth seeks a social worker to provide home-based services across a multi-county area. You will assess needs, coordinate with physicians, and develop care plans for diverse populations in patients’ homes. Strong communication and community resource knowledge are essential.

Applicants should have a Master’s in Social Work and LMSW/LSW licensure, with at least one year in social services. Travel is required; benefits include full day-one coverage and various supports.

Qualifications

• Master's degree in Social Work is required.
• 1+ year experience in social services is required; healthcare setting experience is preferred.
• LMSW or LSW license is required.

Responsibilities

• Utilize the social work process to address physical, emotional, psychosocial, and spiritual needs of patients and families across adolescent, adult, and geriatric populations.
• Review social and emotional factors affecting medical treatment and communicate with physicians and team members for direction.
• Conduct systematic assessments to develop individualized plans with goals and outcomes.
• Identify resources and community agencies to assist patients in accessing care, provide crisis intervention, and offer counseling as needed.
• Document thoroughly and submit records timely; maintain knowledge of community resources and referral processes.
