Job summary

Enhabit Home Health & Hospice is seeking a licensed MSW social worker to provide intermittent professional services, educate patients and families, and coordinate care under physician orders. The role emphasizes adherence to the plan of care and collaboration with the health-care team.

Ideal candidates have a master's in social work (MSW preferred), state licensure, and at least one year of health-care social work experience; basic tech skills and a valid driver's license are required.

Qualifications

• Masters degree in social work required or preferred.
• Must be licensed/certified in the state of practice.
• At least one year of health care social work experience.
• Basic demonstrated technology skills, including mobile devices.

Responsibilities

• Administer social services to patients requiring intermittent professional services and educate patient and family.
• Perform services according to physician orders and the plan of care (POC).

Education

MSW preferred

BSW can work under MSW (Montana/other states)

Certified or licensed in state

1 year social work experience in health care

Basic technology skills (mobile device)
