Job summary

Kaiser Permanente Holy Cross Hospital UR-Discharge Planning in Maryland seeks a qualified Social Worker to collaborate with the care team to coordinate cost-effective services aligned with the member's plan of care, promoting independence and enhanced quality of life.

Essential responsibilities include psychosocial assessment, resource referrals, and care planning. Requires MSW and multi-state credentials within 6 months; onsite, full-time with travel up to 20%.

Qualifications

• MSW required; state credentials from multiple states are required at hire, with additional credentials post-hire.
• Minimum one year of clinical social work experience in a health care setting.
• Knowledge of community resources and regulatory issues for the Mid-Atlantic area preferred.

Responsibilities

• Coordinate with the health care team to plan and monitor care.
• Perform psychosocial assessments to support goals, independence, and quality of life.
• Facilitate access to local/state/federal programs (Medicaid, Medicare) as needed.
• Document case management activities in accordance with standards.
• Schedule family meetings and coordinate care planning with providers and families.
